Abstrak
Through ongoing support across the prenatal, intranatal, postnatal, and neonatal stages, holistic maternity services, also known as continuity of care (CoC), strive to detect problems early. Mrs. Saya, G3P2A0, 35, has been receiving this care at the Karang Rejo Community Health Center since February 27, 2026, when she was 37 weeks and 3 days pregnant. Evaluation results indicated that both mother and fetus were in good condition. The client had attended six antenatal visits according to established standards. A hemoglobin examination revealed mild anemia with a Hb value of 10.7 g/dL and complaints of lower back pain. Education covered the importance of balanced nutrition, adherence to iron tablet consumption, recognition of maternal emergency signs, and childbirth preparedness. The labor proceeded physiologically on March 5, 2026, at 38 weeks and 2 days pregnant, following the standard 60-step APN. The labor lasted 3 hours and 30 minutes, from 6:30 a.m. to 10:00 a.m. WITA (Central Indonesian Time), without any complications. During the expulsive phase (stage II), lasting 8 minutes, meconium was detected in the amniotic fluid, but the baby was born spontaneously in good condition. The baby girl weighed 3200 grams, was 50 cm long, and had an Apgar score of 8/9. The placenta was delivered (stage III) in 8 minutes, between 10:08 and 10:15 WITA (Central Indonesian Time), without complications. Stage IV lasted 2 hours, and observations showed no complications or postpartum hemorrhage. Postpartum monitoring showed the mother's condition remained stable until the 28th day after delivery. Uterine involution occurred with good signs and no infection. The mother was provided with education on nutrition, personal hygiene, breastfeeding techniques, and family planning. The newborn was conscious until 28 days old, exhibited good health, normal reflexes, and was uncomplicated. Education for the baby included nursing care, exclusive breastfeeding, developmental monitoring, and basic immunizations. Overall, continuous care helps maintain the stability of the mother and baby and reduces the risk of complications
